Investigating the Dual Role of SLC7A11 in Hepatocellular Carcinoma Redox Homeostasis and Therapeutic Resistance

Background: Hepatocellular carcinoma (HCC) is the sixth most commonly diagnosed cancer worldwide and the third leading cause of cancer-related mortality, with a 5-year survival rate of approximately 15%, predominantly arising in patients with underlying chronic liver disease. 1 Chronic risk factors including viral hepatitis B and C, alcohol-associated liver disease, and metabolic dysfunction–associated steatotic liver disease drive progressive hepatic inflammation, fibrosis, and malignant transformation through convergent oncogenic mechanisms including NF-κB activation, NLRP3 inflammasome signaling, and viral genomic integration. 1,4,5 Within this chronically stressed microenvironment, HCC cells upregulate the cystine–glutamate antiporter SLC7A11 (xCT) as an adaptive response to oxidative stress, importing extracellular cystine for conversion to cysteine and subsequent glutathione synthesis, thereby conferring resistance to oxidative cell death pathways, including ferroptosis. 2,6 Although SLC7A11 overexpression has traditionally been studied as a pro-survival mechanism, emerging evidence demonstrates that excessive cystine uptake paradoxically depletes intracellular NADPH, promotes disulfide bond accumulation in cytoskeletal proteins, and triggers a recently characterized form of regulated cell death termed disulfidptosis. 3 This context-dependent duality of SLC7A11 — protective at moderate expression levels and lethal at excessive levels under metabolic stress — represents a critical knowledge gap in HCC biology that may have significant therapeutic implications.
Objective: This narrative review investigates the dual, expression-dependent role of SLC7A11 in hepatocellular carcinoma, examining how moderate versus excessive SLC7A11 activity differentially regulates tumor cell survival, ferroptosis resistance, and susceptibility to disulfidptosis, with the goal of identifying exploitable metabolic vulnerabilities for therapeutic intervention in HCC.
Search Methods: A literature search was conducted using the PubMed database from 2016 to 2026 using the following MeSH terms and keywords: “SLC7A11,” “xCT,” “hepatocellular carcinoma,” “ferroptosis,” “disulfidptosis,” “redox homeostasis,” “NADPH,” “glutathione,” “cystine,” “tumor microenvironment,” “metabolic stress,” and “therapeutic resistance.” Given the limited volume of mechanistic studies conducted directly in HCC models, this review additionally incorporated experimental findings from prostate cancer⁷ and lung adenocarcinoma⁸ cell line models in which SLC7A11-driven disulfidptosis pathways have been more extensively characterized, in order to contextualize and inform the mechanistic framework relevant to HCC.
Results: Experimental studies demonstrated that moderate SLC7A11 overexpression enhances cystine import and glutathione synthesis, protecting tumor cells from oxidative stress and promoting growth, whereas high SLC7A11 expression depletes NADPH, drives cytoskeletal disulfide accumulation, and triggers disulfidptosis. 3 In prostate cancer models, super-enhancer–mediated transcriptional regulation of SLC7A11 via FOXA1 was identified, and pharmacologic GLUT1 inhibition with BAY-876 selectively induced disulfidptosis in SLC7A11-high cells by exacerbating NADPH depletion under glucose-limited conditions. 7 Further mechanistic studies showed that glucose deprivation activates p38 MAPK-dependent disulfidptosis in SLC7A11-high cells, and that the protective endoplasmic reticulum stress response (PERK/eIF2α/ATF4 pathway) limits disulfide accumulation; pharmacologic inhibition of this pathway significantly sensitized tumor cells to disulfidptosis both in vitro and in xenograft models. 4 Bioinformatic analyses in lung adenocarcinoma identified fibrinogen alpha chain (FGA) as an upstream regulator of SLC7A11 within the tumor microenvironment, with FGA knockdown reducing SLC7A11 expression, tumor proliferation, and migration — effects partially rescued by SLC7A11 restoration. 8 Clinically, immunohistochemical analysis of 345 HCC patients demonstrated that high xCT expression is an independent predictor of poor prognosis, and tumors co-expressing high xCT and elevated FDG-PET glucose uptake exhibited the worst survival outcomes and highest rates of intrahepatic metastasis, suggesting simultaneous evasion of both ferroptosis and disulfidptosis through metabolic plasticity. 5
Conclusion: SLC7A11 functions as a metabolic double-edged sword in hepatocellular carcinoma: while moderate expression sustains redox homeostasis and confers resistance to ferroptosis, excessive expression creates a latent vulnerability to disulfidptosis that can be exploited therapeutically under conditions of glucose limitation and metabolic stress. 3,5 Clinical evidence confirms that xCT-high HCC tumors represent a biologically aggressive, hypermetabolic subtype that achieves dual resistance to regulated cell death by maintaining high glucose flux to prevent NADPH depletion. 5 These findings suggest that combinatorial strategies targeting GLUT1-mediated glucose uptake and ER stress adaptation may selectively trigger disulfidptosis in SLC7A11-high HCC, offering a novel approach to overcoming therapeutic resistance. 4,7 Future research should investigate the clinical applicability of disulfidptosis-inducing regimens in HCC patients stratified by xCT expression and glucose metabolism, and explore whether the FGA–SLC7A11 regulatory axis represents an actionable upstream target in this disease. 8
